Potrero Medical – A predictive health company helping physicians predict acute kidney injury

Predictive analytics in healthcare provides benefits mainly in clinical care, administrative tasks and operational management. More importantly, the technology is already delivering value in a multitude of healthcare settings, including small private physician practices, healthcare insurance companies and the largest academic hospitals. Moreover, the use of predictive analytics in the healthcare industry is expected to become more prevalent. Electronic health records (EHR) systems already offer many predictive analytics capabilities, and analysts expect vendors to add even more capabilities in the future. Additionally, health IT providers are building their own analytics engines to help healthcare providers deliver optimal care. And they're partnering with healthcare institutions to build proprietary algorithms designed to improve clinical care, administrative performance and operational efficiency.

Potrero Medical is a predictive health company that is developing the next generation of smart sensors and artificial intelligence. Currently, Potrero Medical is developing a predictive health platform to help medical teams better predict adverse outcomes in critical care settings. The Accuryn® Monitoring System is not cleared for prediction of disease states or clinical conditions. Founded in the historical Potrero Hill neighborhood, it emerged out of Theranova, a medtech incubator focused on tackling the biggest challenges in healthcare.  Potrero is a place where the impossible becomes a reality.

Accuryn® Monitoring System

Close to 300,000 people die every year from Acute Kidney Injury (AKI) in the U.S. Critically ill patients need consistent monitoring but without the right tools clinicians don't know when the kidneys are in danger. Studies have shown that intensive urine output monitoring helped detect AKI and improved outcomes. Yet, 74% of all patients do not receive intensive urinary output monitoring due to its difficulty. Airlocks and dependent loops make the situation worse by causing urine to stop and remain motionless in the tubing. Unlike traditional systems, Accuryn automates urine output using patented active drain line clearance. Accuryn's system is also designed to prevent backflow. This allows Accuryn to help prevent retained urine and reduces false oliguria. The Accuryn Monitoring System transforms the traditional Foley catheter into a next-generation sensor for accurate, real-time measurement of urine output (UO), intra-abdominal pressure (IAP), and core body temperature (Temp) to help guide care. The Accuryn Monitoring System provides real-time urine output monitoring that can enable you to detect the risk of acute kidney injury earlier. Automate urine output, intra-abdominal pressure, and core body temperature to recognize clinical patterns and improve workflow. Active drain line clearance helps prevent urine retention due to airlocks and reduce false oliguria readings.

Abdominal Compartment Syndrome (ACS) is a major cause of multi-organ failure in critically ill patients. IAH has a prevalence of at least 50% of ICU patients and is an independent risk factor for death, with 5% progressing to Abdominal Compartment Syndrome. Each time a patient progress to ACS, hospitals lost approximately $598,000 for emergent surgery to save a patient from death. Accuryn's smart sensing Foley has a built-in sensor at the tip of the catheter and can provide real-time IAP measurements all at the simple push of a button. The AccuTab™ can be mounted to an IV Pole to offer maximum flexibility where the monitor resides—in the hall, near the window, outside of the sterile field. Continuous and accurate measurements of urine output, intra-abdominal pressure, and core body temperature.

Accuryn AKI Predict Algorithm

Potrero received FDA granted Breakthrough Device Designation for their AKI Predict machine learning algorithm, for the advanced prediction of acute kidney injury (AKI) associated with intra-abdominal hypertension (IAH) in cardiac post-surgical intensive care patients. Continuous urine output and intra-abdominal pressure are new patient parameters that, until Accuryn, have not been available to leverage as critical data streams. Combining Accuryn technology with a predictive AKI algorithm will enhance the clinical decision support they provide at the bedside. To receive breakthrough designation from the FDA, there are several criteria that must be met. First, the device must provide for more effective treatment or diagnosis of life-threatening or irreversibly debilitating human disease or conditions. Next, the device must meet one of the following: a) it represents breakthrough technology, b) no approved or cleared alternative exists, c) it offers significant advantages over existing approved or cleared alternatives, or d) device availability is in the best interest of patients.

Furosemide Stress Test App

The Furosemide Stress Test (FST) is commonly used in the critical care setting to gauge the likelihood that a patient with early-stage acute kidney injury will progress to more severe stages resulting in major, costly interventions that include readmissions, renal replacement therapy, permanent dialysis and mortality. Traditionally, the FST requires the manual tracking of urine output (UO) over a specific period of time (typically 2 hours), leading to delayed and potentially inaccurate results. The Accuryn Monitoring System now automates the UO tracking portion of the test and logs the result at the exact time prescribed, while advancing workflow for the caregiver team.

In addition to the Furosemide Stress Test, Potrero announced a software upgrade that will launch in unison with the FST App release. The software update enables better patient customization, specifically the ability to enter patient weight down to the 10th of a kg and customize the UO threshold line for each patient.
