The healthcare landscape is undergoing many changes, creating challenges for primary care providers as they work to deliver quality care to the communities they serve. Accountable care organizations (ACOs) are provider and payer arrangements established to improve care coordination between primary care physicians, hospitals, specialists, and public or private health payers. It is expected that ACOs will be capable of reaching quality improvement goals and reducing the costs associated with medical services by bringing greater focus to population health management. The most considerable benefit that it could bring for the healthcare industry is to garner cost savings or reduce spending.

Aledade is one such firm that can help your independent primary care practice thrive by offering personalized practice transformation services, access to meaningful data, and a true commitment to your success. Aledade is a new company with an old-fashioned goal: putting doctors back in control of health care. Aledade partners with independent, primary care physicians to provide everything the doctors need to create and run an Accountable Care Organization (ACO) – from business and practice transformation services to upfront capital and a cutting-edge technology platform. Its customized solutions – and continuous, on-the-ground support of its physician partners – will help doctors in all types of communities across America preserve their autonomy, deliver better care to their patients, reduce overall costs, and keep independent physician practices flourishing.

Path-Breaking Solutions Provided By Aledade

Health Plans: Aledade partners with independent providers, often in historically hard-to-reach rural areas. By establishing value-based contracts, independent practices gain the tools they need to succeed with Accountable Care. Its proprietary population, health management platform, is integrated with more than 90 EHR providers, allowing practices to identify and connect with at-risk and high-risk patients easily. Primary care practices and Aledade only share savings when demonstrating value and reducing the total cost of care.

Health Care Policy: Often when people hear policy, they think of all of the compliance issues ACOs must ensure they are following. Compliance is important, but policy at Aledade is also about analysis and advocacy. The policy team is here to analyze any relevant rules, regulations, and legislation that impacts physician-led ACOs and ultimately advocate for outcomes that work best for its practices.

360° Practice Support: Aledade does not own or manage your practice. They are committed to true clinical and operational autonomy for their partners. The company's role is to provide clarity, simplicity, and support for providers while aligning physician outcomes with patient outcomes. To ensure its partners are prepared for excellence, every member of an Aledade ACO receives the support. Aledade helps turn those pressures into clear and attainable opportunities to deliver better care, strengthen relationships with your patients, and secure long-term sustainability for your practice.

Value-based care: It is a form of reimbursement that ties payments to quality performance, patient outcomes, and appropriate reductions in spending instead of the volume of services provided. The company's goal is to keep patients healthier for longer while curbing the nation's unsustainable spending on low-value healthcare services. In value-based care arrangements, healthcare providers contract with the government or commercial payers to meet certain clinical and financial benchmarks designed to promote proactive, preventive, coordinated care.
