The Silicon Review
Health care management has evolved into a dynamic and complex field with an ability to churn various employment opportunities as the industry continue to grow. This diverse industry is always changing due to scientific discoveries that bring significant contributions to improve the health standards of our communities. The changes have moved on to determine new ways about how and where health care is provided. One of the fastest-growing industries today is the health care management industry. There continues to be a need for comprehensive health care practices because of increasing population as well as the advances in science that have helped to increase the life expectancy of people in most developed countries. There is therefore a need to continually improve the practices involved in health care management to satisfy an ever-increasing market.
Healthmap is a specialty health management company focused on progressive diseases, with a particular expertise in kidney health populations. Health plans such as Medicare Advantage, as well as accountable care organizations (ACOs) and provider groups turn to Healthmap’s value-based solutions to improve the clinical and financial performance for high-risk, high-cost populations. The firm has a rich history forged in data and analytics, having begun as a healthcare data clearing house, adding robust healthcare analytics expertise, and today it uses that experience to power complex specialty health management programs with its clinical experts.
Healthmap’s health management programs: They are designed to meet its clients where they are on the specialty health spectrum, all fully developed, tested and proven in many different healthcare settings. The firm delivers comprehensive specialty health management services, with flexible risk options, comprised of data and analytics, evidence-based best practice clinical guidelines and automated decision-making tools. Its care navigation and provider account teams collaborate with primary care and specialist practices to deliver and help implement overall population health management and individual patient care plans, including a critical medication management component that identifies and flags medication contraindications and recommends course corrections. Its condition care management specialty health programs focus on progressive disease states, with a particular expertise on Kidney Health Management as the leading fully independent solution in the U.S.
Clinical and Financial Data Exchange: Healthmap’s data exchange service simplifies what remains a challenge for most organizations. This simple, yet revolutionary approach to data exchange between internal and external applications gives us the edge when it comes to aggregating data from disparate systems. The firm gathers multiple formats of data, pass it through its data mediation layer, normalize the data, and then enrich the data to pass it to you in an easy to consume and actionable format. This allows you to focus on delivering your value proposition, rather than spending time on data gathering exercises.
Value Based Programs: Applying evidenced based medicine, whether at the point of care for practicing clinicians, or point of approval for managed care organizations, is limited by available data and access to the appropriate evidenced based guidelines and medical policies. Healthmap Solutions gathers data from disparate sources and then feeds that data into its Clinical Decision Support solution which applies the data against a growing library of evidence based medical guidelines and recommendations. Its health coaches reinforce these guidelines through care coordination and interventional support. This level of automated decision making coupled with individual outreach supports a scalable solution that improves the health of the patient, resulting in lower cost of care.
